• Intake Forms will be emailed after booking. Please fill them out before your session — or arrive 10 minutes early to complete in person.

• Customization Form is sent to both new and returning clients so I can tailor each experience that you book. You’ll need to create a login at www.manifesthealingmemphis.com to make changes to the form if desired. Please submit your final preferences at least 24 hours in advance to guarantee I can prep accordingly.
